TIME TO REGISTER!!!!
Dear Parents,
It is time to begin making plans for the 2013-14 school year. Registration of students for next year is an important task for us each spring and this years forms are online for your convenience on the Mesa Verde website: www.amphi.com/schools/mesa-verde.aspx. Select the green “Returning Students” button.
If you are planning a move during the summer and know that your child(ren) will not be attending Mesa Verde for the next school year, please advise your child’s teacher via email or note as soon as possible.
If your child will be attending Mesa Verde next year, we ask that you complete the registration packet online, print and sign the paperwork and return it to your child’s teacher by.
FRIDAY, MARCH 29, 2013.
A completed packet must be returned for each student who will be returning to Mesa Verde next year; otherwise, students WILL NOT be assigned to a teacher for next year.
REQUIRED FORMS:
REGISTRATION FORM – Please fill out completely, indicate grade for next
school year.
STUDENT HEALTH & EMERGENCY CARD – Please list someone OTHER than yourself in case of emergency. We’ll always try to contact you first. Phone numbers and current information is vital when an emergency arises.
PROOF OF RESIDENCY - Please fill out completely, and select ONE of the items indicating your address. We must have a photo copy of the item you indicate as your proof of residency. EVERYONE must complete and provide proof of residency each school year, this is new this year.
EMAIL REQUEST FORM - Please fill out completely.
Please note that this packet should only be used for returning students. If you do not have the ability to print out forms and would like a hard copy please notify the office and one will be sent home with your child(ren).
