This is an order form for the T-shirt style shirts with the same style of printing as last year.

Mesa Verde Volunteers and Parents can show their school pride with our Adult version of the student’s “Spirit T-Shirts.”

Please return this order form to Mesa Verde by

Thursday December 13, 2012, along with $ 15.00 for each shirt ordered.

If you are paying by check please make it out to Mesa Verde Elementary.  Checks are the preferred method of payment - but cash is fine. Please put your cash in a small Ziploc bag and staple it to this order form.

We will contact you as soon as we receive the T-Shirts.
